To help with that, we&#8217;ve added two companion forms:<\/p>\n<ul>\n<li><strong>VA Form 21-4170 (Statement of Marital Relationship)<\/strong> &mdash; used when a veteran or surviving spouse needs to establish a marital relationship that isn&#8217;t otherwise documented.<\/li>\n<li><strong>VA Form 21P-4171 (Supporting Statement Regarding Marriage)<\/strong> &mdash; a corroborating statement from a third party who can attest to the marriage, often filed alongside the 21-4170.<\/li>\n<\/ul>\n<p>Both forms now auto-populate from your case data the same way our other statement forms do &mdash; no more retyping names, dates, and addresses by hand.<\/p>\n<h3>Education and health benefits for dependents<\/h3>\n<p>We&#8217;ve also added two forms tied to dependent-specific benefits:<\/p>\n<ul>\n<li><strong>VA Form 22-5490 (Dependents&#8217; Application for VA Education Benefits, DEA \/ Chapter 35)<\/strong> &mdash; for a dependent applying for education assistance under Survivors&#8217; and Dependents&#8217; Educational Assistance.<\/li>\n<li><strong>VA Form 10-10d (Application for CHAMPVA Benefits)<\/strong> &mdash; for a dependent applying for health care coverage through CHAMPVA.<\/li>\n<\/ul>\n<h3>Add your dependents first<\/h3>\n<p>Both the 22-5490 and 10-10d are generated <em>per dependent<\/em>, so before you can fill them out you&#8217;ll need the dependent on file. Open the veteran&#8217;s case, go to the new <strong>Dependents tab<\/strong>, and add each spouse or child with their relationship to the veteran. Once a dependent is added there, they&#8217;ll show up as a selectable option when you generate the 22-5490 or 10-10d, and the form will fill in automatically using their information.<\/p>\n<p>This is the same Dependents tab we introduced recently for tracking relationship details on a case &mdash; these two forms are the first to plug directly into it, with more dependent-aware forms on the way.<\/p>\n<h3>What&#8217;s next<\/h3>\n<p>We&#8217;re continuing to round out dependent-related form coverage.
